Creators Academy: Digital Design & Technology Club

By submitting this consent form, I confirm that I have parental responsibility for the child whose details I have provided. This consent will expire at the end of the current school year and I can revoke it at any time by contacting Creators Academy.

Club Details

Club Name: Creators Academy Digital Design & Technology Club

Purpose: To explore creativity, coding, art and product engineering using digital applications.

Tools/Applications Used:

• Google Slides

• Google Draw

• Stop-Motion Studio

• Canva

• Artbreeder

• Google Classroom

• TinkerCAD

• PixilArt

• Other (parents to be informed of applications ahead of club activities)

Parental Consent Statement

I hereby grant permission for my child to participate in the Creators Academy Digital Design & Technology Club. I understand and agree to the following:

1. My child will use the specified digital tools and applications as part of the club’s activities

2. Tools and applications not specified above will sometimes be used to facilitate a class. Parents will be informed of the applications to be used ahead of the class and may inform the club if they do not wish their child to participate in activities using those tools.

3. The club will take all necessary precautions to ensure my child’s safety while using these applications.

4. Adult supervision will be provided at all times during club sessions.

5. My child needs to follow the behaviour code (see our Terms & Conditions) and any safety rules so that Creators Academy can keep them and other children safe.

6. I acknowledge that the club cannot guarantee absolute security, but they will follow best practices to protect student data and privacy.
